On I-80 East, take the first Vacaville exit (exit 53) for Alamo Dr. towards Merchant St. Stay to the Left and go up and over the freeway to go straight on Alamo North. Make a Right onto Monte Vista Ave. and follow it all the way down. Turn Left on Cernon St. (Post Office on the corner) and SportsRehab will be on the Right.

From Sacramento:

On I-80 West, take exit 55 for E. Monte Vista/Allison (by the Buffalo Wild Wings) and turn Left onto Monte Vista. Follow all the way down then turn Right onto Cernon St. (Post Office on the corner) and SportsRehab will be on the Right.
